*Overview*
We are seeking a compassionate and dedicated *Licensed Practical Nurse (LPN)* to join our team in the *Long-Term Care (LTC) Rehabilitation* department. As an LPN in our facility, you will play a vital role in providing high-quality nursing care to residents who are undergoing rehabilitation or require long-term care.

*Compensation:*
* *Local LPN*: $34.00 - $40.00 per hour
* *Travel LPN*: $45.00 - $50.00 per hour

*Responsibilities*
* Provide direct patient care, including administering medications, monitoring vital signs, and assisting with daily activities.
* Support rehabilitation efforts by assisting patients with mobility exercises, physical therapy, and other recovery activities.
* Collaborate with a multidisciplinary team, including physical therapists, occupational therapists, and other healthcare professionals, to create and implement individualized care plans.
* Monitor and document patient progress, including any changes in condition, medications, and treatments.
* Educate patients and families on care plans, medications, and lifestyle adjustments to promote recovery and well-being.
* Supervise and guide nursing assistants and other support staff to ensure consistent, high-quality care.
* Advocate for patients’ needs, ensuring that their concerns are addressed and care plans are followed.

*Requirements*
* Valid Licensed Practical Nurse (LPN) license in the state of practice.
* Current BLS certification
* Valid driver’s license
* COVID-19 vaccination (or valid exemption)
