*Responsibilities:*
* Provide high-quality nursing care to patients in their homes
* Develop and implement individualized patient care plans
* Administer medications and treatments as prescribed
* Monitor patient health and report changes to the healthcare team
* Educate patients and their families on health care plans and procedures
* Maintain accurate patient records and documentation

*Requirements:*
* Valid RN license in Massachusetts
* Previous experience in home health care required
* Strong clinical assessment and critical thinking skills
* Excellent communication and interpersonal skills
* Ability to work independently and manage time effectively
* Reliable transportation and willingness to travel within the service area
