The Advanced Practice Nurse (APN) practices as an autonomous and interdependent member of the health care team caring for adults with a variety of cardiovascular illnesses. The APN for the Cardiology Clinic will be expected to practice in the outpatient setting. As a member of this health care team, the APN provides specialized cardiac care to a population of adult patients at Tufts Medical Center through direct nursing care, consultation, collaboration, coordination, and referral. The APN in the Division of Cardiology practices within a collaborative practice model comprised of a physician/nurse practitioner team. The APN works with collaborative physicians and will work with a patient population with a variety of common cardiac conditions and problems, with appropriate cross coverage by all members of APN team as needed.
The APN Practice includes:
Performs a health history and medical history.
Performs and records pertinent health appraisal including physical assessment, laboratory, and diagnostic studies.
Differentiates between normal findings and those requiring treatment, consultation and/or referral.
Assesses the patient’s/family’s psychosocial, emotional, spiritual, and physiologic and environmental needs and priorities.
Diagnoses adults with common acute cardiac conditions, providing treatment and education.
Diagnoses, monitors, and treats patients with chronic cardiac conditions.
Formulates a health plan emphasizing responsibility and participation of patient, family, and other health team members.
Develops collaborative practice model with physician partner(s).
Determines those cases which require appropriate consultation and/or referral to physician or other health care team member.
Provides health maintenance and promotion services through teaching, counseling, and treatment.
Provides symptom management for patients/families along entire disease trajectory.
Identifies resources and coordinates referrals for patients and families requiring further evaluation or services.
Interacts with appropriate community agencies to facilitate implementation of health care team plan.
Collaborates with all members of the health care team.
Prescribes medications for treatment of common adult health and cardiac conditions.
APN will conduct the necessary education to the patient and family for their procedures, testing, medications, and surgery.
Participate in continuing education, quality improvement initiatives, clinical outcome evaluation and monitoring.
Requirements:
Master's degree required
Current MA Registered Nurse license required. BLS required
2-3 years NP experience required
Cardiology NP experience preferred
Current Massachusetts NP licensure
Ability to work collaboratively with nurses, physicians, and administrative colleagues
Excellent communication skills
Ability to navigate complex electronic clinical and administrative information systems and applications knowledge of growth and development
Epic experience preferred
Ability to interpret age specific data and response to care
