Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services. Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in the job specific competencies.nnQualificationsnnGraduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.nnOther advanced degrees (e.g., Master’s, Ph.D.) and applicable Board of Pharmacy Specialties (BPS) or other certification applicable to the position as determined by the supervisor preferred.nnCompletion of pharmacy residency and specialization (e.g., PGY-2 and respective BPS certification), if applicable, preferred.nnCommitment to excellence, accuracy, attention to detail and teamwork required.nnAbility to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.nnEvidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred.nnRelevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred.nnExperience and/or interest in teaching and/or precepting preferred.nnLicense or CertificationnnCurrent pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site’s applicable employee licensure policy.
