Sentara Medical Group is seeking a Flex/PRN Nurse Practitioner or Physician Assistant to join its Palliative Care teams at Sentara Careplex Hospital and Sentara Williamsburg Regional Medical Center in Virginia. The APP is responsible for providing clinical care, support, and education for the palliative patient population as part of the health care team.

Position Highlights:

• PRN/Flex
• Scheduling by mutual agreement, 8 hour shifts between the hours of 7a - 5p
• Performing palliative care consultations and follow up visits for hospitalized patients with acute palliative care needs including symptom management and complex medical decision making
• Facilitating family meetings related to current medical decision making and advance care planning
• Assisting with developing appropriate discharge plans with respect to established goals of care including close collaboration with Hospice for patients to ensure access and continuity of care.
