Job Description

CentraCare is hiring a full-time Family Medicine Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to work with our team-based care model in Long Prairie in Primary Care. 

Responsibilities

As an APP provider (NP or PA) developing your own patient panel in the clinic, you will independently see patients of all ages for acute and chronic care needs.  Our CentraCare – Long Prairie clinic providers diagnose and treat short-term illnesses; provide preventive care including routine checkups, health-risk assessments, immunizations and screening tests; offer personalized counseling on maintaining a healthy lifestyle; and manage and coordinate care for those with chronic illnesses. This position will also support the walk-in clinic, and the physician group and facility needs with nursing home rounds and/or hospital duties as applicable. 

 

Schedule

  • .8-1.0 FTE
  • Option of a four or five-day clinic workweek
  • Rotation into Saturday morning clinic and hospital rounding
  • No call, no evenings

Requirements

 To be eligible for this position you must meet the following requirements:

  • Completion of a formal nurse practitioner or physician assistant program approved by a national accrediting agency
  • Certified as a Nurse Practitioner or a Physician Assistant
  • Current or pending licensure in the State of Minnesota, licensed to see pediatrics through geriatrics
  • BSL Certification; ACLS and CALS

Experience

Position open to new graduates.  Relevant work experience in one of the following areas helpful:

  • Family Medicine
  • Hospital Medicine/Hospitalist
  • Internal Medicine
  • Walk-In Clinic/Urgent Care
