Job Description
  • Coverage Needed:
    • As soon as credentialed - ongoing
    • Rounding: Monday 7a - Monday 7a
    • Hospital call 2 weeks per month
    • Possible afternoon clinic on Tuesday and Thursday
  • Clinical Information:
    • Call responsibilities: Daily hospital rounds, call and clinic time
    • EMR: EPIC
    • Patient Volume:
      • Inpatient 12-15 patients per day
      • Office up to 20 per day
    • Support Staff: 4 RNs & 3.5 MAs
  • Required:
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
    • Active IN License and DEA
    • Board Certified in Hematology/Oncology
