Job Description
  • JOB SUMMARY
    • The Medication History Technician will work primarily in the Emergency Department to assist the pharmacy, nursing and medical staff in obtaining accurate home medication lists from patients entering the hospital and conveying that information verbally or electronically to the appropriate provider of care. The Medication History Technician may also work in Day Surgery, Pre admission testing and on the inpatient nursing units.
  • DUTIES AND ESSENTIAL FUNCTIONS
    • Overall performance rating of below indicated essential job duties:
    • Utilizes a standard process of interview and phone calls to retail pharmacies and physician offices, and may use medication history software and state databases to gather home medication histories
    • Update computerized home medication data bases and reconcile these lists to orders written upon admission to the hospital.
    • Discuss select high alert/targeted medications with the pharmacist, as appropriate.
    • If the medical history was updated AFTER the provider medication reconciliation, notify the pharmacist immediately of any discrepancies between the home medication list and the admission orders.
    • Collects data, conducts quality monitors, and completes documentation as assigned.
  • EDUCATION/LICENSE/CERTIFICATION/OTHER REQUIREMENTS
    • Minimum requirement:
      • High school diploma or GED
      • Certified Pharmacy Technician (CPhT) required or current enrollment in a college of pharmacy (intern)
      • Active Kentucky Board of Pharmacy Technician registration
    • Preferred qualifications:
      • Two years of previous pharmacy technician experience (1-2 years of retail experience considered positive)
  • WORKING ENVIRONMENT
    • Constantly works near moving mechanical parts
    • Constantly exposed toblood and/or body fluids andinfection.
    • Occasionally exposed to toxic or caustic chemicals
    • Occasionally exposed to risk of radiation
    • The noise level in the work environment is usually moderate.
  • PHYSICAL DEMANDS
    • Occasionally required to maintain in a stationary position (standing or sitting).
    • Frequently required to walk and move about.
    • Frequently required to operate a computer and other equipment.
    • Frequently required to use fine motor skills to handle and operate instruments/devices.
    • Constantly required to lift and/or move items up to 10 pounds.
    • Frequently required to lift and/or move items up to 50 pounds.
    • Occasionally required to lift and/or move items up to 100 pounds.
    • Constantly required to communicate with patients and other care team requiring talking and hearing.
    • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, and depth perception.
    • Occasionally required to position self in order to meet patient needs.
