The successful candidate will have 5 yrs of Inpatient multi-unit/hospital wide discharge planning experience that includes 1 or more years working in a level 1 or large academic medical center
Must have EPIC charting experience
Must have compassion and empathy but be able to communicate quickly and effectively.
