The Patient Navigator position at Pinnacle will allow you to engage with the patient throughout their journey while working closely with a multidisciplinary team to provide excellent patient care.
We are seeking a Patient Navigator to join our dedicated team at Seattle Reproductive Medicine in your preference of location Bellevue, Kirkland or Seattle WA. This is a full-time position, fully onsite, working 8:30am - 5:00pm with one required holiday per year and four weekends per year, with an additional 2 on-call shifts.
RESPONSIBILITIES
- Serve as the primary point of contact for patients undergoing Third Party Reproductive services.
- Guiding patients through their treatment plan and providing education on procedures, testing, and follow-up care.
- Coordinate Third-Party Reproduction (TPR) treatment based on the provider's treatment plan and under the supervision of the registered nurse navigator, providing patients with next steps.
- Participate in cycle-specific visits and daily rounding by relaying and clarifying provider instructions regarding medication instruction (schedule, dosage, and administration) and follow up appointments for TPR-Assisted Reproductive Technology (ART) cycles (with the exception of trigger/pre-op instructions).
- Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
- Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
- Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
- Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
- Educate patients on treatment protocols, self-care strategies, and medication usage.
- Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
- Advocate for patients' needs and help resolve any issues or barriers to care.
- Other duties and project as assigned.
POSITION REQUIREMENTS - Bachelor's degree in healthcare, social work, or a related field preferred or a Certified Medical Assistant Certification (CMA) preferred.
- At least 1 year of fertility coordination, to include ART cycles. Third-Party Reproduction cycle management and/or coordination is preferred.
- Minimum of 2 years of experience in customer service, patient navigation, case management, or a related healthcare role.
- Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
- Excellent communication, organizational, and problem-solving skills.
- Proficiency in using electronic medical records (EMR) and other healthcare software.
- Empathetic, patient-focused, and able to work collaboratively in a multidisciplinary team.
- Bilingual skills are a plus.
COMPENSATION & BENEFITS $28.00 - $38.00 hourly or annually
The compensation range for this position is between $28.00 and $38.00 per Hour. Final salary offers will be determined based on factors such as years of experience, skills, and qualifications.
