Job Description & Requirements Specialty: Gerontology Discipline: Nurse Practitioner Start Date: ASAP Duration: 11 weeks 40 hours per week Shift: 8 hours, days Employment Type: Local Contract Role and Responsibilities: Patient Assessments: Complete thorough clinical assessments for each resident on at least a monthly basis, with increased frequency for high-risk patients. Care Coordination: Collaborate with an RN who assists with coordinating care, ensuring timely follow-ups, communication, and continuity of care across facilities. Primary and Preventive Care: Provide primary, preventive, and urgent care on-site, managing both chronic and acute needs. Personalized Goals of Care: Facilitate conversations around advanced illness, setting personalized goals for each resident. Gaps in Care: Identify and address gaps in care related to STARS, HEDIS, and COHPS measures, recommending adjustments to the primary care provider when necessary. Medication Management: Prescribe or refill non-narcotic medications after consulting with the resident’s primary care provider. Care Transitions: Offer single visits to patients transitioning from the hospital to the SNF, assessing and preparing them for a safe and successful discharge.
