DESCRIPTION:

We are seeking a compassionate and dedicated Certified Home Health Aide to provide personalized care to clients in their homes. RESPONSIBILITIES:

• Promote and restore patients' health by following the care plan
• Collaborate with Community Health Nurse/Therapist
• Perform personal care, light housekeeping, meal preparation, and errands
• Maintain effective communication with office and supervisory staff
• Accurately and completely document client care
• Adhere to required department and system protocols and regulations

REQUIREMENTS:

• Home Health Aide certification (HHA) - New York State Education Department (NYSED)
• Valid New York State Driver's License (NYS DL) - New York State Department of Motor Vehicles (NYSDMV)
• Ability to exert physical force for medium work and frequent movement
• Refer to program-specific requirements for additional experience and educational expectations

PAY:

Base pay range: $17.60 - $19.50 per hour
