Position Overview

The Intake Specialist will;
n
n - Screens and assesses applicants applying for emergency shelter, SUS/Palladia Veterans Services & Homebase.
n - Serves as the primary initial SUS contact point for Veterans and their families.
n - Navigates applicants for program services through intake process, documentation standards and necessary supports.

ESSENTIAL JOB FUNCTIONS
n
n - Schedule and conduct intake and follow-up appointments with program applicants, including screenings via telephone, in person, and in the field at Department of Homeless Services (DHS) installations, homeless shelters and other outreach/recruitment locations.
n - Understand and communicate the programs’ eligibility and admission criteria.
n - Demonstrate sound understanding of Homebase and Veterans Division programs.
n - Assess households’ immediate service needs, providing referrals when necessary for immediate services.
n - Document intake screenings and maintain records for each applicant and enrolled participant in both DHS CARES and AWARDS systems.
n - Collect required documentation for program enrollment and scan documents for digital archiving.
n - Assist applicants with obtaining necessary documentation for program enrollment.
n - Notify Veteran households of intake decisions.
n - Provide referrals for Veterans services both in-house and for external supports.
n - Develop strong linkages with referring workers and agencies.
n - Assemble intake reports including spreadsheets to summarize data.
n - Participate in team meetings.
n - Prepare formal letters and enclosures for distribution to applicants and referral sources.
n - Respond to information requests from internal SUS staff and DHS.
n - Maintain close contact with participants and staff.
n - Participate in SUS Veterans Service Fair(s) and other company events.
n - Completion of other tasks as assigned by manager.

Qualifications
n
n - Bachelors’ degree or higher preferred.
n - Out in the field approximately 75% of the time throughout Nassau and Suffolk County.
n - Required to meet Veterans out in the field, either using their personal vehicle or the company vehicle. A clean driving record is required is using a company vehicle.
n - Veterans are highly encouraged to apply.
n - Willingness to attend occasional events outside of normal business hours.
n - Ability to work on Veterans Day.
n - Former service in the U.S. Armed Forces, past experience with working with U.S. military veterans and/or high level of military cultural competency is preferred.
