The Fred Finch CCTAY program provides comprehensive services for young adults or Tranisition Aged Youth (TAY) delivered by a team comprised of compassionate, committed multidisciplinary staff. The program's primary goals are to assist young adults in achieving their recovery-oriented goals, help them obtain and stabilize in housing, and build self-sufficiency skills to help them function effectively in their homes and communities.
In line with the Fred Finch core values of respect, compassion, and hope, CCTAY staff treats each participant with dignity, compassion, mutual respect, and unconditional high regard. The CCTAY program draws from evidence-based and evidence-informed best practices to provide comprehensive, individualized services: The Assertive Community Treatment(ACT) model informs how the multidisciplinary team operates and functions, while the Transition to Independence (TIP) model and Motivational Interviewing informs our interventions and treatment planning process. Individually designed services support participants to gain the skills, knowledge, and abilities to move successfully into adult life. Through their engagement in program services, participants learn to imagine and develop aspirations for the future. Our staff work with participants to establish a collaborative relationship and develop a service plan that centers around demonstrated needs, motivation, and attainable life goals. Staff provides a wide variety of supportive services designed to promote participant stability in their own community: mental health services; medication support; vocational/employment support and skills building; and educational and health linkages are all important elements of a comprehensive treatment.

What You Will Do
The Parent/Family Partner provides direct care to identified parents and caregivers in a community or home setting. Family Partners may also provide direct care to youth or young adults. The primary role of the Parent/Family Partner is to provide one-on-one support and mentorship to parents and caregivers, and/or participants or other family members, by sharing experience and expertise in navigating systems and programs (e.g., Regional Center, Children and Family Services, Special Education Systems, Juvenile Justice Systems, Placement Services, Behavioral Health Care Services, or Residential Centers). The Partner works collaboratively with the parent, caregiver, or participant and other appropriate community and treatment team members to provide the best therapeutic care possible. Along with the rest of the team, the Partner is active in supporting and implementing an individualized, evidence-based, culturally competent, family-centered approach.

Salary Range In accordance with California law, the expected salary range for this California position is between $18.61 and $2375 per hour. The actual compensation will be determined based on experience and other factors permitted by law.

What You Bring to The Table (requirements):
  • Experience advocating, preferably as the family member, for someone who has received mental health or other services. Parent Partner: Experience as a parent who has received services for a special needs child.
  • Possess good communication skills, including active listening.
  • High School Diploma or GED required.
  • Demonstrated ability to utilize de-escalation techniques that align with the trauma informed care model as part of intervention strategy for working with participants in crisis who are responding to triggers/stressors and/or demonstrating early indicators of distress.
  • Demonstrated cultural responsiveness in working with diverse families and
  • Basic computer and keyboarding competence with the ability to use Microsoft Office Suite and/or competency with electronic clinical documentation
  • Current CA Driver's License with a clean driving record or ability to obtain one within 10 days after hire.
  • Ability to work a flexible schedule including weekends.
  • Ability to work as part of a collaborative, multi-disciplinary mental health team.
  • Must have access to dependable transportation and personal car insurance, meeting minimum liability coverage required for position if driving for work or transporting participants.
Preferred
  • A.A. or B.A. Degree.
  • Prior parent mentoring experience.
  • Experience in group facilitation skills.
  • Familiarity with local communities.
  • Case management, familiarity with symptoms of mental illness, and service documentation/record keeping experience.
  • Fluency in prevailing language(s) of the community.
