Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.
LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.
The Physician provides primary care and continuous medical coverage, and directly provides services to LIFE Program participants. Demonstrates the knowledge and skills necessary to assess, plan care for, and provide medical care to frail elderly clients according to assigned responsibilities and LIFE standards.
Center hours are 8:30 a.m. to 5:00 p.m. and includes rotating night and weekend call with other LIFE primary care providers.
QualificationsPhysician must possess current M.D. or D.O. license from the state of Pennsylvania and a graduate of an accredited school of medicine.
Have at least one year experience with a frail or elderly population.
Physician must have working knowledge of current community health practice for the frail elderly from direct service experience, and have a working knowledge of the interdisciplinary model of care management.
Experience in internal or geriatric medicine in a managed care environment preferred.
Physician must have experience in working with cognitively impaired seniors, and must be able to relate well with seniors and their families to deal with sensitive issues and facilitate problem solving.
Working knowledge of computers, Microsoft Word and Excel required.
