  • Monitor and assess patients' ongoing conditions, adjusting treatment plans as necessary in collaboration with the attending physician.
  • Discharges; provide discharge instructions, medication management, follow-up appointments, and any lifestyle modifications or dietary recommendations
  • Communicate findings and treatment recommendations to the referring team, ensuring integrated care for the patient
