The CNA memory care support works on a collaborative interdisciplinary including Social Workers, RNs, Volunteer Coordinators, and Clinical Assistants.

HOW YOU'LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Memory Care Support CNA is responsible for patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, documenting care. This position is also responsible for coordinating and facilitating activities specific to Memory Care Residents in collaboration with the Interdisciplinary Team. The Memory Care Support CNA provides patient and family-centered care consistent with Agrace's philosophy, goals, standards, regulations, and policies.
  • Assist Patients with Activities of Daily Living
    • Assist with ambulation, changing position, and moving to/from sitting and standing.
    • Help with bathroom functions like toileting and incontinence care.
    • Help bathe, shower, dress, provide oral care, and groom.
    • Assist with eating and drinking.
    • Perform post mortem care.
    • Obtain vital signs. Assist with treatments.
    • Anticipate and respond to patient needs and round on patients.
  • Provide homemaker assistance
    • Perform light housekeeping such as changing linens, doing laundry, tidying room, and removing trash.
    • Deliver patient supplies.
    • Deliver food and beverages.
  • Communication
    • Read, understand, and follow the plan of care. Document cares and changes in patient condition in electronic health record.
    • Document and report significant changes in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
    • Reinforce patient and family education.
  • Miscellaneous (Percentage of Time 10%)
    • Participate in meetings to plan and evaluate patient/family services.
    • Precept new staff and students.
    • Delegate tasks to volunteers as appropriate.
YOU BRING (EXPERIENCE & QUALIFICATIONS)
  • High school diploma or GED, or equivalent combination of education and work experience
  • Graduation from an accredited Certified Nursing Assistant program
  • Current, unrestricted Nursing Assistant Certification in state of employment
  • One-year personal care experience, hospice, or home health, preferred
  • Experience with working in a Memory Care facility, preferred
  • Must be at least 18 years of age
  • Ability to develop, maintain, and apply knowledge of hospice and end-of-life process
  • Ability to maintain professional boundaries with patients, families and co-workers
  • Ability to follow plans of care, and patient, family, and clinical staff directions
  • Basic Microsoft Office skills
  • Experience using electronic health records; Epic preferred
  • Ability to work effectively and cooperatively with others
  • Ability to communicate effectively verbally and in writing
  • Ability to be flexible in time and task to meet organizational needs
  • Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
  • Ability to use proper body mechanics, safe patient handling, and standard precautions
