Remote but will be servicing community via Home Visits in Bexar county.

Job Duties:

• Determine needs and eligibility of members for various services through field-based collaboration with medical, behavioral, and social support staff. Conduct screening and risk assessment interventions per program guidelines.

Performance Expectations/Metrics:

• 3 business day turnaround time for documentation submission, timely follow up with members on new service/items requested, 75% travel required.

What previous job titles or background work will in this role?

• Service Coordination, Case Management or Nursing experience

Pay Rate: $20 -$25 per hour

Hours : Monday Friday 8 AM 5 PM
