Your Impact
- Serve as the primary point of contact for patients, caregivers, and providers, helping facilitate seamless transitions of care and addressing any barriers along the way.
- Conduct initial assessments to understand patient needs, goals, and any obstacles they may face in accessing care.
- Develop personalized care plans in collaboration with healthcare teams, tailoring support to meet each patient's unique needs.
- Coordinate appointments, referrals, and tests to ensure patients have timely access to the care they require.
- Advocate for patients to make sure they receive necessary treatments, medications, and resources to support their health journey.
- Provide education to patients and caregivers on health conditions, treatment plans, and effective self-management practices.
- Track patient progress and outcomes, adjusting care plans as needed to promote the best possible health results.
- Accurately document patient interactions, care plans, and interventions in electronic health record (EHR) systems.
- Work with insurance providers and connect patients to community resources to address social determinants of health and ensure access to necessary supportive services.
- Participate in care conferences and team meetings to discuss patient goals, progress, and discharge planning.
- Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization's Values.
Your Credentials - Passion for healthcare and a strong desire to support patients in navigating their care journey.
- Bachelor's degree in a healthcare-related field preferred (e.g., Public Health, Health Administration, Social Work) or equivalent experience in a healthcare setting.
- Excellent interpersonal and communication skills, with the ability to build rapport with patients, caregivers, and healthcare providers.
- Strong organizational skills and attention to detail, with the ability to manage multiple tasks and coordinate appointments and services efficiently.
- Basic knowledge of healthcare terminology and an understanding of healthcare workflows.
- Familiarity with electronic health records (EHR) systems is a plus, with the ability to learn new software quickly.
- Ability to work effectively in a team-oriented environment, collaborating with multidisciplinary teams to support patient care.
- Problem-solving skills and the ability to advocate for patient needs.
- Flexibility and adaptability to handle a dynamic healthcare environment.
- Experience with insurance coordination and knowledge of social determinants of health is a plus, but not required for entry-level applicants.
