Job Title: Care Navigator

Position: #00827391 - Requisition: #34982

Job Summary:

This position is based in the Infectious Disease Group Practice at the University of Colorado Hospital. The Care Navigator provides basic case management and retention outreach activities for HIV and PrEP patients, including navigating patient billing issues. This is a hybrid role.

Key Responsibilities:

  • Provides basic case management services to HIV patients in addressing identified barriers.
  • Provides retention outreach activities for PrEP patients.
  • Support care navigation activities; including assisting patients with referrals to other support services when appropriate.
  • Acts as the billing liaison for HIV and PrEP programs as part of the patient support team.
  • Troubleshoot patient billing issues by working with patients, respective billing departments, and relevant insurance companies.
  • Act as liaison with UCHealth and CU Medicine billing departments.
  • Ensure patient demographic and insurance information updated in the electronic medical record (EPIC).
  • Assist with insurance verification as needed.
  • Assist patients with insurance navigation if indicated.
  • Work as part of the multidisciplinary patient support team; including participating in team meetings.
  • Proper documentation of all patient encounters in the electronic medical record, and other databases as applicable.

Work Location:

Hybrid - this role is eligible for a hybrid schedule of 3 days per week on campus and as needed for in-person meetings.

Qualifications:

Minimum Qualifications:

  • Bachelor's degree in any field.

A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year for year basis.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:

  • Bachelor's degree in human services field.
  • One (1) year of case management or related experience.

Anticipated Pay Range:

The starting salary range (or hiring range ) for this position has been established as $56,556 - $60,000 .

The above salary range (or hiring range ) represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
