Pay Range = $ 17.55 / HRnnThe Home Health Aide (HHA) is a member of the home care team who works under the supervision of a Registered Nurse or Therapist. The HHA performs various personal care services, as necessary, to meet the patient’s needs. The HHA provides service in accordance with Maxim’s philosophy, standards and policies.nnEssential Duties and Responsibilities:nnObserves and documents patient condition and reports any changes, to the assigned nurse supervisor.nnCommunicates in a constant, clear, and effective way with the client, family/caregiver and officennResponsible for assisting patient with and per plan of care, the following;nnBathsnnBack rubsnnOral hygienennShampoosnnChanging bed linennnDressing and undressingnnSkin care to prevent breakdownnnToileting activitiesnnKeeping patient’s living area clean and orderly, as appropriatennPlanning and preparing nutritious mealsnnFeedingnnIncontinence carennPractice basic infection control standardsnnChanging the patient’s position in bed or wheelchairnnTaking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency)nnSafe transfer and ambulation techniquesnnExercise according to the plan of carennPerforming range of motion and other simple procedures as an extensional therapy service as ordered (with appropriate completed/demonstrated skills competency)nnSelf-administration of medicationnnPatient’s laundrynnMeeting safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.)nnObservation and teaching of home safety practicesnnRecognition of an emergency situation and knowledge of emergency management proceduresnnPractices within scope of certificate/licensennMust follow patients plan of care and complete all tasks assignednnAdherence to Maxim’s policies and procedures, documentation, care procedures, and standards of personal and professional boundaries and conductnnAbility to live by Maxim’s company-wide commitment to quality and competent patient carennResponsible for completing all assigned trainings in a timely mannernnPerforms any other duties as assignednnMinimum Requirements:nnMeets all training requirements in accordance with state and federal laws when applicablennAbility to read, follow written instructions and document care givennnPossess and maintains current CPR certification in accordance with state regulationnnPPD or chest x-ray if applicablennSuccessful completion of Maxim screening examinationsnnParticipation and completion of Maxim’s competency programnnMust meet all federal, state, and local requirementsnnAbout Maxim Healthcare ServicesnnMaxim Healthcare Services has been making a difference in the lives of our patients, caregivers, employees and communities for more than 30 years.
