What you will do: - Provides annual Medicare wellness visits for FPG patients
- Assist in pre-visit planning and identification of eligible patients
- Is familiar with Medicare requirements and maintains compliance with policy updates
- Will work collaboratively with the care management team and refers patients for care coordination and health coaching when appropriate
- In collaboration with patient and healthcare team, establishes individualized, objective action steps that are time specific to assist the patient in improving their overall health
- Provides ongoing support and evaluation of progress toward stated health goals. Revises the healthcare plan as needed in collaboration with patient and healthcare team
- Facilitates patient medication management based upon standing orders and protocols.
- Acts as an advocate for patient and helps them to obtain their maximum level of health
- Provides education to patients about their specific conditions and needs
- Possesses excellent communication and collaboration skills
- In collaboration with ordering primary care provider, helps to coordinate needed referrals, testing and closure of gaps in care
- Maintains EHR documentation and data. Completes all documentation in a timely manner
- Participates in medical home model of care and value-based care initiatives
- Interacts harmoniously with others and contributes to an overall sense of wellbeing within the office setting
- Focuses on the attainment of organizational goals and objectives and displays a commitment to teamwork
- Is flexible when needed to assist care team with other patient care duties as assigned
- Participates in Care Management team meetings and quality improvement initiatives.
- Participates on a team for data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Medical Home initiatives.
- Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
- Labeling, documenting, and tracking all specimens or tests ordered/received following set standards and reporting all test results to the physician.
- Charting care coordination on day of visit to ensure documented, organized and chronological medical record with the submission of timely, accurate and complete information to the Central Billing Office.
- Assisting with scheduling appointments, tests or procedures.
- Answers phones in a pleasant manner, screens calls following clinic guidelines to appropriately and expeditiously direct caller.

What you will need: - Graduate of an accredited program of professional nursing and possess a current license as a Registered Nurse from the State of Ohio.
- Currently certified in BLS and maintain certification.
- 3 years experience in a clinical setting required with chronic disease management and adult preventive care knowledge demonstrated.
- Proficient computer skills, including Microsoft Office (specifically Word and Excel)
- Highly organized and well-developed oral and written communication skills
- Demonstrates sound judgment, decision-making and problem-solving skills
- Interpersonal skills such that employee can develop effective relationships with medical and administrative staff, patients, co-workers and the public.
- Ability to remain calm and poised in urgent situations.
- Skill in exercising initiative, judgment, problem solving, and decision making.
- Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
- Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of emergency codes.
- Ability to comply with OSHA regulations and CDC standard and transmission based precaution recommendations and to utilize proper personal protective equipment
- Ability to comply with provisions of applicable S.D.S. forms.
- Successful completion of a 90 day get-acquainted period.
