n Duties: Job Description:n-The staff Registered Nurse (RN) is a professional caregiver who assumes responsibility for a group of patients for a designated time frame and provides care to these patients via the utilization of the nursing process, nursing trends, technology, financial and human resources within the philosophy of the institution.nnEssential Functions:n-Performs all responsibilities and duties required by unit as specific to the scope of service.n-Implements procedures and theories related to the specific area of practice.n-Maintains up-to-date and accurate documentation of nursing care provided to assure the integration of information for use by the healthcare team.n-Completes initial assessment upon patient admission and develops appropriate plan of care in accordance with unit standards and that is based on patient and family needs.n-Implements plan of care, nursing interventions, and patient care procedures.n-Modifies plan of care based upon continuous evaluation.n-Takes responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents.n-Communicates with others directly and in private when necessary to resolve issues.n-Offers constructive feedback to assist in the professional development of peers, and confers with colleagues to expand knowledge base.n-Identifies own learning needs and takes initiative to begin appropriate solution.n-Fulfills all requirements of departmental competencies.nnDepartment Specific Job Functions:n-Completes initial assessment upon patient admission to the Rehab unit and develops appropriate Rehab plan of care in accordance with unit standards and is based on patient and family needs.n-Implements Rehab plan of care, nursing interventions, and patient care procedures.n-Modifies plan of care based on continuous evaluation.n-Coordinates nursing care activities in collaboration with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall patient goals.n-Implements procedures and theories related to Rehab practice and scope of service.n-Maintains up-to-date and accurate documenting of nursing care provided to assure the integration of information for use by the healthcare team.n-Shares information about the disease processes underlying disabilities and teaches nursing techniques to help the rehab patient and their families develop self-care skills necessary to move toward wellness.n-Participates in Rehab rounds and team meetings.n-Reinforces the teaching done by therapy in rehab, provides resource materials for patient’s changing needs, and provides on-going patient education.n-Possesses the specialized knowledge and clinical skills necessary to provide care for people with physical disability and chronic illness.n-Fulfills all requirements of Rehab departmental competenciesnSkills: Required Skills & Experience:n-Minimum three (3) years of relevant nursing experience.nnPreferred Skills & Experience:n-N/AnKeywords:nEducation: Required Education:n-ASNnnPreferred Education:n-BSNnnRequired Certifications & Licensure:n-Illinois state RN License in good standing -BLS (American Heart Assoc.)n
