COMPANY OVERVIEW: Alvita Care is a mission-driven Home Healthcare & Personal Care Agency with four branches and over 500 caregivers. We provide relief and assurance to family members in the areas of In-Home Personal Care, Alzheimer's & Dementia Care, Palliative & Hospice Care, and Geriatric Care Management and have earned The Joint Commission Gold Seal of Approval. Alvita Care clients are all private-pay and we do not accept Medicare or Medicaid.
As we expand our circle of care, we are seeking top-notch caregivers to service our Westchester and Rockland clients. We have private-duty one-on-one care cases available as well as facility based care cases.
All applicants are subject to a phone screening to determine compatibility and overall availability of accommodations for both the caregiver and the agency. Please note that cases with Alvita Care are only available to caregivers who successfully complete the onboarding process.
DETAILS: - We have clients throughout Westchester requiring shifts from 8hrs to 12hrs .

- Shifts are assigned to provide 1:1 care in the home environment
QUALIFICATIONS: - NYS RN licensure.
- BLS certification.
- Minimum of One (1) year demonstrated experience as an RN
- Ability to put patients at ease and explain physician's instructions.
- Ability to multi-task with excellent organization skills.
- Experience working directly with people from diverse racial, ethnic and
socioeconomic backgrounds. - Knowledge of medical terminology.
- Team player and ability to work independently
- Valid driver's license or NYS Non-Driver photo ID card, required
- Drivers preferred!
ESSENTIAL DUTIES & RESPONSIBILITIES: • Assessments needs of the clients to develop a plan of care
• Consult with Management on complex/unique cases
• Provide a plan of care with support of the Clinical/Medical/appropriate
multidisciplinary members to ensure the services follow the plan of care by assessment, planning, coordination, counseling, teaching and supervision to meet needs of the clients/patients/.
• Monitor/Assess patients/clients' condition, effects of the care plan,
• Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals.
• Manages, plans, coordinates and executes the patient's safe discharge from service.
• Ensures that physician's orders, discharge summary and communication to team members, patient and family members adhere to company and
regulatory requirements.
• Communicates and consults with patient's MD with all manners when needed, manage and coordinate all aspects of assigned caseload.
• Assists patients with activities of daily living
• Documents content and process of all services provided in internal system properly.
• Perform other related duties and participates in special project, as assigned.
