We are currently seeking a Registered Nurse Case Manager to join our rapidly growing team in Westchester County. As the RN Case Manager, you will be responsible for developing and maintaining client-specific care plans, ensuring these plans are followed, and providing for the client’s ongoing needs in a private pay, concierge level home care. This role requires 70% fieldwork for a small caseload of clients demanding great attention to detail.

The salary for this full-time position ranges from $110,000 to $130,000, commensurate with related experience.
n
QUALIFICATIONS:
n
n- Current NYS RN license in good standing
n- BSN preferred
n- Minimum of two years recent clinical experience within a Home Care setting
n- Strong assessment and organizational skills
n- Strong time management skills
n- Prior supervisory experience with field staff
n- Proven track record of working independently
n

PRIMARY RESPONSIBILITIES:
n
n- Ensuring the highest quality of client care
n- Developing care plans
n- Obtaining necessary physician orders for home care cases as required by State regulations
n- Reporting changes in the client’s medical or mental condition to the attending physician and the Agency’s team involved in the client’s care
n- Supervising and supporting the home care team
n- Striving to improve the lives and sustainability of our clients by supporting their autonomy in the home environment
