Description
Provides primary and episodic care in collaboration with an identified physician to patients of all age groups in any and/or all departments.
Primary Responsibilities
- Performs initial visit patient assessments
- Conducts triage, health promotions, health screening, episodic, acute and chronic problems
- Diagnoses and manages patient presentation for triage, episodic, acute and chronic problems
- Recognizes, coordinates and revises management plans for conditions throughout the life span
- Orders and interprets laboratory and diagnostic tests and procedures
- Performs EKGs
- Institutes selective screening procedures as indicated
- Acts as primary responder and provider in emergency situations
- Facilitates patient wellness through health promotions, education and follow up
- Refers patients for inter- and intra agency services, community resources and supportive services
- Performs psychosocial assessment in order to understand factors that influence patient's health status as indicated
- Involves patients' significant others in appropriate decision-making processes as indicated
- Acts as patient advocate
- Participates in monthly staff meetings
- Participates in and acts on recommendation from clinical record review
- Participates in relevant in-services
- Completes all requests from schools, camps, exams for certificates, and return to work forms
- Collaborates and coordinates patient visit needs with LPN's and M.A.s
- Precepts nurse practitioner students using the health center as a clinical site as desired
- Participates in orientation of new staff members
- Completes all documentation as assigned and in an appropriate time frame
- Finalizes patient encounter forms and notes and/or dictation within 8 days of visit
- Responsible for clear, consistent, up-to-date documentation of all interactions with patients
- Independently seeks physician and supervisory consultation
- Seeks ongoing growth/development resulting in maintaining and securing active licensure
- Maintains current CPR certification
- Participates in performance improvement activities
- Effectively and accurately delegates nursing and non-nursing activities
- Participates in at least one committee on an annual basis
- Assumes any other tasks and duties as delegated in a timely and coordinated manner

Qualifications
A Board Certified Nurse Practitioner with a Bachelor's Degree and a Master's Degree in Nursing
Current, active, professional licensure to practice in an expanded nursing role in the Comm. of MA
Eligible for DPH and DEA controlled substances registration within two months of employment
One year nursing experience, preferably in a community health setting
Telephone Required
Acts only within the scope of their state license
Able to demonstrate knowledge of the principles of growth and development over the life span
Able to assess data reflective of a patient's status and interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs
Flexibility to negotiate job-share positions requiring constant coverage
