The
Licensed Practical Nurse is responsible for utilizing the nursing process to provide for optimum quality and continuity of nursing care to patients in the comfort of their residence.

Responsibilities • Complete and scan required record keeping forms/charts upon the guest's admission, transfer, and/or discharge.
• Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest's response to the care.
• Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Director of Nursing.
• Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest's medical record in accordance with established procedures.
Qualifications • Current, unencumbered LPN/LVN license required or Compact State license- required.
• A minimum of 2 years LPN/LVN experience required.
• Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
