DescriptionnnPay $30.00 - $31.50 an hournnThe Licensed Practical Nurse (LPN) is responsible to provide daily nursing care, order medications, provide updates to the RN and Nurse Practitioners, train and orient AMAP’s, schedule appointments, participate in agency activities and complete required documentation.nnJoin the Upstate Caring Partners Team as a Licensed Practical NursennOpportunities available working with adult or youths.nnMonday - Friday days only.nnAbility to pick up 24 hours a month (12 off hours/off-shift during weekdays & 12 off hours during weekends)nnPaid TrainingnnStarting pay $30/hour!nnSign-on Bonus - up to $3,000nnCore ResponsibilitiesnnPromote the individuals’ independence by implementing the Individual Program Plan.nnAttend to personal, self-care and other program participant needs.nnProvide support and consultation and initiate peer support among program participants.nnPromote program participant input in the development of program activities.nnOversee the safety and well being of the program participants.nnOversee and upkeep the maintenance of equipment and supplies.nnParticipate in activities as part of the treatment team.nnEnsure compliance with all pertinent government and agency regulations and operating standards.nnQualificationsnnA.A.S. Degree or High School plus Specialized Trade Certificate.nnValid NYS Licensure as a Licensed Practical Nurse and current NYS Registration Certificate.nnStrong interpersonal and communication skills.nnBasic Computer Skills (Windows, Outlook, Adobe Acrobat, Word, Excel)nnTravel is required.
