Position Summary: We are seeking a Ful-Time dedicated and compassionate Home Hospice RN Case Manager to join our team in Green Valley, AZ and surrounding areas. The ideal candidate will have a strong background in hospice care and be committed to providing high-quality, patient-centered care.
**$5,000 bonus**
Key Responsibilities:
- Provide direct patient care in the home setting, managing symptoms and addressing patient and family needs.
- Develop and implement individualized care plans based on patient assessments.
- Coordinate with interdisciplinary team members, including physicians, social workers, and chaplains, to ensure comprehensive care.
- Educate patients and families on disease progression, symptom management, and end-of-life care.
- Monitor and document patient progress, making adjustments to care plans as necessary.
- Participate in on-call rotation to provide support to patients and families as needed.
Qualifications:
- Active RN license in the state of Arizona.
- Minimum of 1 year of hospice nursing experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Compassionate and empathetic approach to patient care.
- Valid driver's license and reliable transportation.
