Licensed Practical Nurse (LPN) Full-time / Overnights $33.40-$34 - $1000 bonus
Advancing Opportunities, an affiliate with Apis Services is currently seeking qualified candidates to fill the role of Part-time Licensed Practical Nurse who delivers health services to individuals with disabilities who are experiencing mental and physical conditions.
The Licensed Practical Nurse will utilize the nursing process documentation and communication skills to collaborate with physicians and other team members. Supervise provisions of consumer services. Provides nursing assessments and care plans to Advancing Opportunities consumers. Provides health education training as assigned, participates in planning and treatment planning meetings. Keeps abreast with professional developments in nursing and related areas. Maintains an effective system of records and reports. Oversight of medical information, and medication training for clients and staff, as appropriate. Performs other duties as from time-to-time, be deemed appropriate by the supervisor.
Qualifications & Requirements
- High School diploma or equivalent. Graduated from an accredited Licensed Practical Nursing program.
- Current State of New Jersey LPN license.
- Ostomy and Wound Care experience
- Minimum of one year of experience providing nursing services to individuals with disabilities.
- Valid driver's license
- Consent to State & Federal Criminal Background Checks;
- Drug Screen; Pre-Employment Physical & PPD;
- Complete all orientation & pre-service training as required
Required Skills and Abilities: - Must have experience with Ostomy and wound care, Bolus feedings, G-Tubes, and Suprapubic Catheters
- Knowledgeable about specific state laws and regulations governing practice and continually updates knowledge base.
- Able to multi-task and switch priorities throughout the day.
- Excellent writing skills, and the ability to present facts and recommendations effectively in oral and written form.
- Able to accurately calculate medication dosages.
- Excellent interpersonal skills and ability to communicate effectively with individuals with disabilities, families, co-workers, staff, supervisors, funding sources, and other social service agency personnel
- Ability to solve problems effectively
- The ability to work independently with minimal supervision
Essential Responsibilities : - Provide prescribed treatment and health care procedures as ordered by a physician.
- Advises supervisor of all concerns and/or issues related to the provision of nursing services.
- Create and distribute Individual Service Plan Medical/Health Reports detailing participant's general health, health care goals, and physician recommendations for each participant.
- Document on the individual's health status for various support people including family, guardian, staff, employment and other professional staff as necessary.
- Records a detailed report of care given which reflects the therapeutic plan and specific aspects of the patient care plan on individual's official electronic health record.
- Monitor the consumer's use of medications through Medication Administration logs.
- Make referrals for needed medical services and confirming that follow-up care is pursued.
- Instruct all new support staff on consumer specific medical needs.
- Provide quarterly in-service training for support staff and Managers.
- Monitor weight gain or loss and other health conditions of participants.
- Ensure MAR's are being followed through regular reviews and unannounced checks
- Maintain health services supplies inventory and places supply orders when the inventory is low.
- Submit IR's and URI's for medication errors or other medical concerns with assistance from Managers.
- Provide medical training to staff as needed.
- Perform other job related duties as assigned
Physical Requirements: Mobility and ability to bend and reach during an eight-hour day. Able to lift a minimum of 25 pounds. Able to sit approximately 2-3 hours per shift, stand or work 6-8 hours per shift, and drive a company van 1-4 hours per shift. Has visual and auditory acuity sufficient to evaluate and treat a client needs. Fine motor skills and legible and accurate writing of reports, daily correspondence and presentation either manually or orally. Ability to work an eight-hour day.
