Strong Children Wellness is a network of primary care practices integrated into community-based organizations that serve children and families. We focus on a comprehensive healthcare model that addresses medical, behavioral, and social needs all in one place. Our innovative “reverse integration” model allows us to provide personalized, multidisciplinary care teams that support the physical, developmental, emotional, and social needs of our families.
We are expanding! We’re opening a new location in Woodside, Queens, and increasing our operational days at our existing location in Jamaica, Queens. We are looking for a dedicated and skilled Licensed Practical Nurse (LPN) to join our growing team.
Purpose of the Position:
As an LPN at Strong Children Wellness, you’ll play a critical role in providing high-quality patient care and being an essential member of our clinical team. You will work under the direction of a Physician or Nurse Practitioner to deliver general nursing care to patients and support a seamless healthcare experience.
This role is scheduled to work 9:00 am to 5:00pm every Friday at our Woodside location. We may need some additional availability to work at our Jamaica location on Tuesdays. Please apply if you have availability for either of these days.
Requirements
What You’ll Do:
- Assist physicians and nurse practitioners with day-to-day office operations and patient care.
- Provide excellent care to all patients and demonstrate courtesy during patient interactions.
- Ensure that pre-visit planning is completed and help identify gaps in care.
- Accept and manage patient assignments and assist in patient examinations.
- Administer specified immunizations and medications as ordered by the provider.
- Observe patients and promptly report adverse reactions to medication or treatment.
- Document all services performed accurately for billing purposes.
- Schedule tests or treatments at other facilities and forward medical information as needed.
- Participate in team huddles and actively share knowledge and ideas.
- Communicate frequently with providers about patients’ physical and mental conditions.
- Perform routine laboratory test procedures following established guidelines.
What You’ll Bring:
- Education: High school diploma or GED; One-year LPN certificate from an accredited college or technical school.
- License: NYS LPN license.
- Experience: Experience working with both adult and pediatric patients.
- Skills: Strong clinical competence, effective communication, adaptability, and the ability to function as an effective team member.
