• June 14th
• June 28th
• September 27th
• December 13th
• December 27th

Job Details:

• Setting: Outpatient adult cancer patients
• Hospital Outpatient Department, Cancer Center, & Radiation Oncology
• EMR System: EPIC and Mosaiq documentation systems

Estimated credentialing timeframe: ~90 days
Requirement description :

• Board Certified - REQUIRED
• Active OH licesne & DEA - HIGHLY Preferred, willing to consider IMLC
• BLS - REQUIRED
• Flu Vaccine - REQUIRED at time of credentialing
• COVID Vaccine - Preferred
• Preference to candidates available for ALL listed dates

Required Procedures:

• Conventional Radiotherapy
• 3D Radiotherapy
• Photon Beam Radiation Therapy
• Stereotactic Body Radiation Therapy (SBRT)
• External Beam Radiation - 3D-CRT
• External Beam Radiation - IGRT
• External Beam Radiation - IMRT

CERTIFICATION REQUIREMENTS :

• BLS
• Board Certified

STATE LICENSE REQUIREMENTS :

• Ohio

ADDITIONAL LICENSE REQUIREMENTS : Willing to consider candidates needing to obtain OH license for May dates only

On Call Requirements : No
