Delivers nursing interventions based on professional judgment in response to individualized patient needs. Assist with the admission process by collecting patient and family information. Promotes patient engagement by considering specific needs, values and preferences. Assists with discharge teaching about diagnosis, medications, and self-care skills to prevent hospital readmissions. Accurately and completely documents patient information and nursing care in the patient's medical record. Promotes continuity of care by reporting activities, incidences, and changing patient needs to next caregiver. Demonstrates infection prevention behaviors, consistent hand washing and use of appropriate PPEs. Adheres to safe medication administration while preparing, administering, documenting and monitoring meds. Recognizes safety risk and implements prevention plan for falls, skin breakdown and injury. Contributes to the department's performance improvement activities. Demonstrates competency in performance of LPN clinical skills.nnEssential Skill Competencies: Phlebotomy, Establish IV access (LPN II only), CPR Certification, Medication monitoring, Preventive skin care, Foley catheter insertion/care, Aseptic wound care/ dressing change.nnMinimum Requirements: Current LPN license by the Mississippi Board of Nursing.
