You will be assigned to a location to perform Face-to-Face (F2F) encounters and be responsible for gathering clinical information to support the hospice diagnosis and prognosis. These findings are then communicated to the hospice physician responsible for certifying or recertifying the patient's hospice eligibility.nnThe scope of practice for the hospice NP is limited to those tasks necessary to complete hospice F2F encounter visits.nnPerforms and completes physical assessments for the F2F encounter within the regulatory time frames established by CMSnnProvides clinical findings, including review of the medical treatment plans, diagnostic tests, lab results, and medication lists to the physician who is recertifying the patient for hospice eligibilitynnMakes appropriate recommendations to the IDG based on findings of the F2F assessmentnnNotifies hospice agency when unable to complete a F2F visit according to schedulennAbout YounnMaster's or higher degree in Nursing.nnCompletion of a graduate-level nurse practitioner education program accredited by a national accrediting body.nnCurrent state license as a registered nursennCurrent state license or certificate as an advanced practice nursennCurrent national certification as a Nurse Practitioner.
