Job Title/Position: Hospice On-Call Nurse
Reports to: Branch Clinical Manager
Position Summary:
The Hospice On-Call nurse will partner with our local and regional leadership as well as the Interdisciplinary Teams to administer patient care as needed while on-call.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care:
  1. Spend quality one-on-one time with patients and their families to support everyday needs.
  2. Conduct comprehensive OASIS assessments and care plan development.
  3. Provide skilled nursing care as outlined in the plan of care.
  4. Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
  5. Practice knowledge of Medicare CoP's, proficient documentation skills, attention to detail, prioritization, and time management.
  6. Evaluate effectiveness of services provided to the patient, consulting with physicians as needed.
  7. Ensures their charting in the Electronic Medical Record is consistently finished in a timely manner in accordance with the current agency practice.
  8. Monitor clients' conditions; reporting changes to Clinical Manager and Physicians.
  9. Follow up with, executing, and properly documenting doctors' orders.
  10. Case management and care coordination while in the field with support from the clinical manager.
  11. Assure that care is in accordance with Federal and State guidelines.
  12. Full time minimum visit expectation per established metrics as defined in offer letter.
  13. Be on-call at all times during work day.
Communication:
  1. Utilize effective written and verbal interpersonal communication skills.
  2. Ensure all personnel involved in client care have current clinical information and current orders.

POSITION QUALIFICATIONS
1. Registered nurse with current licensure to practice professional nursing in the state of practice. Demonstrated ability to supervise and direct professional administrative personnel
2. Experience in case management, disease management, home health care nursing, hospital nursing or intensive out patient education.
3. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
SKILLS REQUIRED
1. Organized and resourceful self-starter, strong ability to work in a team.
2. Excellent observation, verbal, written and interpersonal communication skills.
3. Pull, push and or lift 15-20 pounds on a daily basis. Pull / push 125-200 pounds on a regular basis. Pulls/push 200 or more pounds on a regular basis
• Sits, stands stoops, squats, walks, and moves intermittently during work hours.
• Drives in own car to and from patients' home. Work in patient(s) homes as well as agency office.
• Ability to cope with the mental and emotional stress of the position.
