Direct Care To ensure that all individuals are treated with respect, dignity and support to take as much control as possible over routines activities in their home. To teach, assist and support individuals in the community and at home so that the individual is able to learn, practice, and master skills that enable him/her to enhance his/her status by assuming socially valued roles. To support each individual’s existing relationships and increase their networks of friends and acquaintances. To ensure that all individuals are supported to explore and express their interest for community membership. To participate in individual advocacy system. To maintain neatness and cleanliness of individual’s bedrooms. To maintain regular contact and a positive relationship with individual’s family members/guardians, which includes providing status updates, scheduling visits, and acting as a positive liaison between family/guardian and the residential program. To maintain regular contact and a positive relationship with work program staff. To assist the individuals with household shopping and maintenance as necessary, providing ongoing opportunities for individuals to assume increasing control and responsibility. B. Staff Supervision and Program Leadership Assist with supervising staff directly and act as a team leader. Lead teams by example through role modeling and coaching. Assist with actively promoting and positively role modeling the philosophy, mission, and core values of the organization. To take a leadership role in advocacy with and on behalf of the individuals. To facilitate teamwork between all team members via open communication, sharing of responsibilities and a positive, constructive attitude. Provide on-call coverage as assigned/trained. C. ISP and Care Plan Implementation To implement ISP/Support Plans and strategies as written and at frequency stated in the plan. To monitor and maintain individual’s ISP program book to ensure data collection occurs at frequency stated. To review data collection monthly and prepare written progress notes quarterly or at frequency stated in ISP to assist in the development of ISP assessments. To train staff in the implementation of intervention strategies and correct data collection procedures. To ensure that the health and wellbeing of individuals are met by accompanying individuals on appointments as needed, administering medications (or supporting them to self-medicate) in accordance with residential guidelines and DDS policy. D. Crisis Management Respond effectively to potential safety hazards and emergencies. To initiate contact with supervisors as needed in the event of concerns and/or crises. E. Administrative Duties To assist with safeguarding sensitive and confidential data. To participate in supervision meetings with the Residence Director. To assist with completing financial documentation. To perform additional duties as required or assigned. Qualifications Required Qualifications: 18 years old High School Diploma or equivalent Valid Driver’s License & Good Driving Record Prior direct care experience Preferred Qualifications: Proven record of conscientious, responsible behavior (work, volunteer, or school history) Prior supervisory experience preferred Medication Administration Program (MAP) certification preferred Basic computer literacy/proficiency preferred BA degree or current enrollment Thank you for your interest in Seven Hills Foundation We value every applicant.
