Job Title: Per Diem RN Visit Nurse - Home Care and Community Health
Company: Home Health Care, Inc.
Schedule Option #1: Saturdays Only (can be every other)
Schedule Option #2: 1 Weekday of your choice and 1 Weekend per month.
Pay: $40 - $44 per visit.
Mileage reimbursement between clients!
Job Summary
We are seeking compassionate and dedicated Visiting Nursed to provide high-quality healthcare services to patients in their homes and community settings. The ideal candidates will possess a strong understanding of patient care, with the ability to assess and monitor vital signs, administer treatments, and educate patients and their families about health management. This role requires excellent communication skills and the ability to work independently while collaborating with healthcare teams.
Duties
  • Conduct comprehensive assessments of patients' health status, including vital signs and medical history.
  • Administer medications and treatments as prescribed by physicians.
  • Provide wound care, infection control, and other necessary medical interventions.
  • Educate patients and families on disease management, medication administration, and health maintenance.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Monitor patient progress and report changes in condition to the healthcare team.
  • Assist with home community care services, ensuring a safe environment for patients.
  • Maintain accurate documentation of patient care activities in compliance with healthcare regulations.
Qualifications
  • Registered Nurse (RN) license in good standing.
  • Strong knowledge of anatomy, internal medicine, infection control practices, and critical care transport protocols.
  • Experience in home community care settings is preferred.
  • Familiarity with sonography, ophthalmology procedures, CT scans, and dialysis processes is a plus.
  • Ability to perform vital sign assessments accurately.
  • Excellent interpersonal skills with a focus on patient-centered care.
  • Strong organizational skills and attention to detail.
  • Ability to work independently while managing multiple patient cases effectively.
