SUMMARY

As a Licensed Practical Nurse, you will play a key role in providing compassionate care to residents in long-term and post-acute care. You will lead the care team, guide CNAs in delivering excellent patient care, and ensure proper documentation is always maintained. Your leadership is vital for both our staff and the wellbeing of our patients.

The Clifton Springs Nursing Home, located next to Clifton Springs Hospital, provides a holistic approach to long-term care with a 108-bed facility offering access to unique wellness treatments such as massage therapy and acupuncture.

STATUS: Part-Time

LOCATION: Clifton Springs Nursing Home

DEPARTMENT: Long Term Care

SCHEDULE: Evenings

RESPONSIBILITIES

  • Patient Care & Service: Promote and restore patients' health through the nursing process; collaborate with the medical team; perform treatment procedures; provide emotional, educational, and physical support to patients and their families; supervise supportive staff.

  • Documentation: Maintain complete and accurate resident care documentation using computerized medical record systems.

  • Support for the CNA Team: Direct CNAs in timely and quality personal care, ensuring accurate and thorough documentation.

EDUCATION:

LICENSES / CERTIFICATIONS:

BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)

PHYSICAL REQUIREMENTS:

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; requires frequent walking, standing, or squatting.

PAY RANGE:

$28.50 - $34.50
