Key Responsibilities:

• Ensure effective, outcome-focused nursing care is delivered by following the nursing process.
• Develop and adjust individualized care plans in collaboration with residents and the interdisciplinary care team.
• Utilize available resources for effective discharge planning and resident/family counseling.
• Deliver optimal care by staying updated with current nursing practices and demonstrating essential skills.

Qualifications:

• Active LPN license in the state of practice.
• Current CPR certification; must be obtained within 90 days of hire.
• Experience or education focused on geriatrics is preferred.
• Able to maintain regular attendance and a reliable work schedule.
