Key Responsibilities:
- Ensure effective, outcome-focused nursing care is delivered by following the nursing process.
- Develop and adjust individualized care plans in collaboration with residents and the interdisciplinary care team.
- Utilize available resources for effective discharge planning and resident/family counseling.
- Deliver optimal care by staying updated with current nursing practices and demonstrating essential skills.
Qualifications:
- Active LPN license in the state of practice.
- Current CPR certification; must be obtained within 90 days of hire.
- Experience or education focused on geriatrics is preferred.
- Able to maintain regular attendance and a reliable work schedule.
