Licensed Practical Nurse The
Licensed Practical Nurse is responsible for, but not limited to:
- Communicate completely at all times. Listens attentively and clarifies understanding. Explains what is happening.

- Through the continuous improvement of standards of practice and by providing a safe and secure environment the patient will be assured a sense of safety and well being, free from risk and harm.
- Attendance/Tardiness: Contributes to the effectiveness of the Agency by remaining within the acceptable standard of the Agency's Attendance/Tardiness Control Policy.
- Is responsible for own actions and for seeking patient information necessary to function effectively in assigned health setting. Maintains personal competency.
- Assists in assessment of patient needs, implements and documents nursing care under the direction of the RN.
- Observes the patient's condition and reports changes of condition and the need for nursing measures beyond his/her knowledge.
- Is an active member of the health care team assisting the Physician and/or RN in performing specialized procedures by preparing equipment and assisting in treatments while utilizing aseptic techniques.
- Is capable of writing legible, accurate clinical and progress notes regarding the patient's care and condition.
- Will assist the patient in personal care along with basic nursing care.
- Assists the patient's family and other health care team members in providing continuity of care and meeting the emotional needs of the patient and immediate family.
- Continually updates professional knowledge and skills through attendance and participation in continuing education and in-service classes.
- Performs all skills, treatments and procedures completely and within the regulations and licensing laws of the State Nurse Practice Acts, ACHC's policies and procedures and other applicable local/federal laws.
Education: High School Diploma or equivalent
Professional Experience: - Current LPN license within the State of New Jersey
- 1 year of Community Health Nursing
