POSITION SUMMARY
The RN Case Manager provides comprehensive nursing care to assigned patients, assessing their and their families' needs, developing and implementing care plans, and adjusting them as necessary based on changing patient conditions. The RN Case Manager will also identify needed interdisciplinary services.

STATUS: Full Time
LOCATION: Ontario/Seneca County, with travel required to Yates and Cayuga counties
SCHEDULE: Monday-Friday Day Shift
IDEAL CANDIDATE ATTRIBUTES:
Associate Degree in Nursing is required; 1 year of nursing experience is preferred.
Prior home health, clinical, and direct patient care experience is a plus.
Ability to work independently within a community setting.
KEY RESPONSIBILITIES:
Patient Care & Service: Collaborates with patients and their families to create an interdisciplinary Plan of Care based on assessed needs.
Planning: Manages and coordinates care for patients, ensuring efficient and effective service delivery.
Communication: Effectively communicates changes in patient status to the care team in a timely manner.
Documentation: Maintains accurate records of patient care and coordination activities per agency standards.
Compliance: Adheres to privacy regulations and demonstrates respect for patient confidentiality.
EDUCATION & EXPERIENCE:
Must meet qualifications based on department-specific care programs and continuing education requirements.
PHYSICAL REQUIREMENTS:
This role involves medium work, regularly exerting 20 to 50 pounds of force, and frequently 10 to 25 pounds. It requires regular walking, standing, and squatting.
COMPENSATION:
$36.40 - $48.50 per hour
