Job Description
  • Coverage Needed:
    • First available dates below, will have additional dates through 2025
      • May 5-12, 2025
    • Shift: 7am to 7am
  • Clinical Information:
    • Covering Emergency Department and Hospital consults and emergencies
    • Only ED and Hospital work, no office
    • Cover emergencies and any needing surgery
    • Patient volume: 0-10 daily
  • Required:
    • CV must be in Month / Year format
    • Must have license in hand at time of submission, or proof that the license application has been submitted
    • Any gaps greater than 30 days will require an explanation on the CV
    • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
    • Current Flu Vaccine
    • Must be fully vaccinated for Covid 19
      • Exemptions only allowed for religious or medical reasons, once approved by the facility.
    • Active IN License and DEA (or easily transferrable DEA)
    • EPIC experience
    • Board Certified in Urology
    • Must be available to work 5/5/25-5/12/25
    • Required Procedures/Common Diagnosis: Hydronephrosis with renal and ureteral calculous obstruction, Calculus of kidney, Benign prostatic hyperplasia with lower urinary tract symptoms, Calculus of ureter, Urinary tract infection
