Position Overview:
As a Nurse Practitioner or Physician Assistant, you will be responsible for providing high-quality, safe, and cost-effective care to patients in local post-acute care communities. You will work with an interdisciplinary team to ensure comprehensive patient care.
Responsibilities:
- Patient Rounds: Conduct H&P, admissions, progress notes, transfers, discharges, and acute/follow-up care as required by CMS.
- Care Coordination: Collaborate with the facility to transfer patients to the most appropriate level and site of care upon discharge, including home, independent living centers, assisted living, and memory care.
- Interdisciplinary Collaboration: Work closely with hospice, palliative care, and home care agencies to ensure a safe transition home.
- Team Support: Receive support from local and regional operations teams.
Requirements:
- Active Nurse Practitioner or Physician Assistant license
- Experience in post-acute care or a similar setting preferred
