Position: Chief of Palliative Care 

The Monogram Health is seeking a full-time physician, specialized in palliative care, to join the team due to rapid nationwide opportunities to treat members with CKD and ESRD.  This position will be responsible for clinical aspects of the palliative program including quality improvement, education, and ensuring high-quality, patient-centered care for individuals with life-limiting illnesses, while collaborating with other health care providers to coordinate comprehensive care plans and advocate for patients and their families, while working closely with operations leaders for clinical input to program development. This physician also provides consultation to patients with CKD and ESRD appropriate for conservative care and symptom management and engagement with patients who have multiple complex co-morbidities.   

Monogram physicians are quality-driven physicians who apply their clinical expertise in a managed care, population health context. By practicing at Monogram Kidney Clinic, they serve patients across the country through a national telehealth platform. They also engage in population health management in a sophisticated, outcomes-driven manner that promotes palliative care where clinically appropriate and focuses on patient education, autonomy, and quality of life. The Chief of Palliative Care should be experienced in treating patients to reduce or control pain and debilitating symptoms and in driving discussions about goals of care, prognosis, treatment options, and advanced care planning.    

This Monogram physician may work remotely from home. As part of working at Monogram’s national practice, they will be licensed in multiple states after joining the company. Monogram physicians work on an employed basis, full time, five days per week.  

Roles and Responsibilities 

  • Participation and clinical leadership in the clinical development, implementation, and evaluation of all palliative care programs and services? 
  • Establish quality standards and metrics for palliative care delivery? 
  • Develop of clinical metrics to demonstrate program impact 
  • Collaborate with operational leadership to integrate palliative care into clinical practice 
  • Hiring of Palliative Care Physicians 
  • Spokesperson for the program internally and externally 
  • Provide direct patient care to complex palliative care cases, including symptom management and advanced care planning 
  • Apply evidence-based criteria in determining the appropriateness and necessity of care; in particular, in determining whether dialytic or nondialytic care is the correct evidence-based pathway  
  • Consult with other healthcare providers regarding patient care and complex cases.? 
  • Lead interdisciplinary team rounds to discuss patient needs and coordinate care across disciplines. 
  • Reduce or control pain or debilitating symptoms on patients who have selected a palliative care pathway  
  • Reduce unnecessary or preventable episodes of care, including hospital readmissions  
  • Assist in the training of NPs and other providers to provide high quality care   
  • Other duties as assigned  

Position Requirements  

  • 3 of clinical experience beyond board certification and 1 year of administrative experience; developing a palliative program, preferred 
  • 1+ year treating patients with CKD and ESRD  
  • 1 year of experience working as part of a multidisciplinary team 
  • Management and leadership experience 
  • Licensing Requirements 
  • Current state medical license without restrictions to practice and free of sanctions from Medicaid or Medicare.  Willingness to become licensed in multiple states as company and patient needs require.   
  • Education Requirements 
  • Doctorate Degree in Medicine  
  • Board Certified in internal medicine and/or geriatric medicine, hospice, and palliative medicine
