Key Responsibilities Provide high-quality, compassionate care for patients in long-term care settings, focusing on geriatric nursingCollaborate with nursing staff, medical directors, and specialists to create comprehensive care plans tailored to geriatric patients.Accurately document visits in an electronic medical record (EMR)Support acute and chronic care management, ensuring patient needs are met efficiently and effectively as an Advanced Practice Nurse. Qualifications Required Qualifications: Active Virginia Medical License and DEA LicenseMinimum one year of clinical nursing experienceStrong communication skills, self-motivated, adaptable, and a team player Preferred Qualifications:Nursing experience in ICU, Med/Surg, Emergency Medicine, or Hospitalist rolesExperience with care coordination and working in long-term care settings, especially geriatric nursing COVID-19 Compliance To protect the health of our workforce and patients, all employees must disclose their COVID-19 vaccination status before employment.
