DescriptionnnPeaceHealth is seeking a LPN Home & Community for a Part Time, 0.60 FTE, Day position. The salary range for this job opening at PeaceHealth is $27.47 – $41.21. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.nnHiring bonus and relocation assistance may be available.nnUnder the supervision of an RN, the LPN provides direct nursing care, within the scope of their license, to patients as directed by the physicians and other healthcare providers. Cooperative decision making is expected in the execution of duties.nnDETAILS OF THE POSITION:nnImplements and evaluates patient care under the supervision of an RN. Participates in the development of a patient-specific care plan.nnProvides direct patient care in patient homes including administering medications and treatments as prescribed. Understands and applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post-surgical drains, and pulmonary and abdominal drains.nnProvides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed.nnDemonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting.nnDocuments patient care in health record according to department standards.nnWHAT YOU BRING:nnGraduate of an accredited State Board of Nursing LPN Program.nnRegistered as a LPN with the state of Oregon.nnCurrent Basic Life Support (BLS) certification issued by the American Heart AssociationnnValid Drivers License and proof of InsurancennPeaceHealth is committed to the overall wellbeing of our caregivers: physical, emotional, financial, social, and spiritual.
