This is a mobile health wellness assessment position traveling from home to home - PRN and no guarantee of hours/assessments per week
The Group
Matrix Medical Network is a leading provider of home-based and mobile health care services ranging from wellness exams to advanced diagnostic tests. Matrix partners with health plans and providers across the country to provide their Members with free annual health assessments, medication reviews and preventative screenings that complement care from their doctors. Matrix licensed and credentialed health providers include Physicians (MD/DO), Nurse Practitioners, Physician Assistants and medical technicians. They conduct exams and screenings in the convenience of a Member's home or at a Matrix mobile health clinic nearby
The Position
Type of clinic/specialty: LOCUMS - Mobile Healthcare for Medicare (mostly) and Medicaid (infrequently) patients
IDEAL CANDIDATE - is a NURSE PRACTITIONER WITH AT LEAST 3 Months Experience and has an appreciation for the culture of caring for this patient population and great bedside manner
This is an annual health/wellness assessment - no bloodwork, no prescribing. When conducting a home assessment, the provider should ask questions that cover various aspects of the patient's health, environment, and overall well-being. In the unlikely event of an emergency, the provider should arrange for emergency care
How many hours per week: 6 assessments per day 2 Shifts per week avail requested
Typical Work Hours: Each Assessment is roughly 45 - 60 minutes
Whom else will the provider be working with?, is this a solo position: Solo
How many patients per day: up to 6 if scheduled (PRN work)
Call schedule: No call, but schedule is released Beginning of the month, provider provides their availability, then they go into LUMEN on Tablet (Medical Record/Charting system and Scheduling) and they plan their day according to patient availability.
EMR system: Tablet provided - Chart for now but transitioning to LUMEN
PAID TRAINING - 20ish HOURS OF ON-LINE MODULES - $400

Compensation and benefits
Salary range: $100/Assessment
Travel Stipend: $60 each day up to 120 miles traveled or $300 each day for over 120 miles traveled - that is if they're traveling far enough that it's an overnight.
Interview & Offer Process - CV and Candidate sent to Tamina.
PLEASE have all candidates and presents include ALL licensure, certifications and if they are set up with Medicaid. Candidate CV and Present submitted through ICIMS portal, reviewed by Jessica Hobson, then submitted to Team Lead or Lauren Renner, NP. They conduct the 30 minute phone interview with very specific questions on examples of cases and how provider would treat, if feedback is favorable, they make the offer. VERY SIMPLE
We will have our credentialing - documents provided by Emily and Matrix has their required documents as well. Matrix can get the provider on the road in two weeks if the following is completed:
