Job Summary

Supervisory responsibility for all disciplines and coordination of services provided at our White Rock Center for the Prisma Health SeniorCare PACE Program. The program provides a quality and comprehensive approach to long term care needs of families who wish to avoid institutional care and have the ability to manage the needs of their loved one in a community setting. The foundation for service delivery is the PACE center which is a hybrid between a physician practice, outpatient rehab facility and medical day care. This position has administrative responsibility for daily clinical activities, project delivery and direct operation of our smallest PACE center Less than 20 FTEs and 80 participants. On-Call is required.

“Prisma Health SeniorCare PACE is an outpatient setting.  Schedule is Monday through Friday with daytime hours.  Prisma Health SeniorCare PACE offices are closed on holidays and weekends.”

Essential Functions

  • All team members are expected to be knowledgeable and compliant with Prisma Health's values:  Inspire health.  Serve with compassion.  Be the difference.

  • Coordinates the delivery of health care in the home and in the PACE center through the supervision of staff responsible for ensuring participant needs are met 24 hours per day, seven days per week.

  • Coordinates all center activities including care planning and implementation, preparation for interdisciplinary conference and daily meetings, family conferences and all related follow up.

  • Directly and indirectly supervises center staff, including nurses, therapists, social workers, transportation, and administrative staff.  Responsible for hiring, performance review, and other related human resource duties.

  • Operates the center within established operating budget expectations and provides analysis.

  • Documents participant changes appropriately in the medical record and communicates participant changes to team members.

  • Serves as liaison between individual program disciplines, and community service providers.

  • Leads team meetings daily and participates in PACE Leadership team monthly and serves as a liaison between administration and direct staff.

  • Performs other duties as assigned.

Supervisory/Management Responsibility

  • This is a supervisor job which may have direct supervision of team members which may include hire/termination authority, disciplinary authority, and performance management responsibilities. May have budget input or responsibilities. Job is not considered a member of management staff.

Minimum Requirements

  • Education - Associate degree in Nursing. BSN preferred.

  • Experience - Four (4) years of experience in the field of nursing, social service, sociology, psychology, or in an area closely related to health and social development for the aging including one (1) year of working with the frail or elderly population. Supervisory experience preferred
