As a Case Manager, you will monitor and manage patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. You will be part of a robust and caring multidisciplinary team caring for our patients from admission through transfer or discharge. Required Skills:• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.• Must be able to work in a stressful environment and take appropriate action.• Demonstrates proficiency in verbal and written communication skills.• Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external.
