We are currently seeking an experienced Registered Nurse (RN) Case Manager to join our Care Continuum Specialist team, focused on Assisted Living Facilities (ALFs). This position will cover the Central Fairfield County region (Wilton, Redding, Weston).

Position Summary As part of the Care Continuum team, the RN Case Manager is responsible for overseeing, coordinating, and providing high-quality nursing care to patients within Assisted Living Facilities (ALFs), ensuring seamless transitions between care settings and fostering long-term relationships with referral sources. This position plays a critical role in helping RVNAhealth ensure excellent patient outcomes, and promoting positive relationships with ALF staff, families, and clinicians.

Key Responsibilities and Duties
n
Perform comprehensive patient evaluations and develop personalized care plans in collaboration with patients, families, physicians, and other healthcare providers.
n
Conduct accurate and timely Oasis assessments.
n
Identify patient and family needs for additional resources or home health services and make appropriate referrals.
n
Lead case conferences, team huddles, and facility meetings to ensure coordinated care and improved patient outcomes.
n
Ensure that all necessary documents, including Memorandums of Understanding (MOUs), are up to date and compliant with regulations.
n
Provide education and support to patients, families, and staff regarding treatment plans and home care management.
n
Supervise Home Health Aides and Licensed Practical Nurses (LPNs) as needed, ensuring effective communication and documentation.
n
Monitor patients’ progress, communicate changes to physicians, and ensure proper documentation of all care activities.
n
Regularly attend team meetings, in-services, and educational opportunities to stay current on best practices and regulatory compliance.
n
Work within the Business Development Team to increase awareness of RVNAhealth services among strategic referral sources and foster relationships within the ALF community.
n
Ensure smooth transitions of care for patients leaving the facility and entering home care services.
n
Participate in continuous quality improvement initiatives and maintain compliance with agency standards, regulatory guidelines, and HIPAA.
n
Experience and Qualifications
n
Bachelor’s degree in Nursing from an accredited institution.
n
Current Registered Nurse (RN) license in Connecticut.
n
Minimum of 3 years of clinical experience, with at least 1 year of home health experience.
n
Experience working in Assisted Living Facilities (ALFs) or similar settings.
n
Familiarity with the Oasis assessment tool and home health regulatory guidelines preferred
n
Familiarity with home care principles, community health administration, and practices in the home setting.
n
Strong interpersonal skills and the ability to establish and maintain collaborative relationships with patients, families, and interdisciplinary teams.
